Your email address will only be used for our automatic reminder system that informs you when your pet’s vaccines, recheck appointments, and any other pertinent information regarding your pet, are due. This method will be used in lieu of reminder cards or phone calls.
*All medications require a doctor label with dosage instructions.
I hereby authorize the veterinarian to examine, prescribe for, and/or treat the above described pet. I assume full responsibility for all charges incurred for the care of this animal. I also understand that these charges will be paid at the time of release and that a deposit may be required for surgical treatment. I authorize VIP Animal Hospitals to use photographs of my pet for purposes of publicity, advertising, web content, and/or the VIP Facebook page.
All animals being boarded by VIP Animal Hospitals must be current on all vaccinations. Canine patients must have an up to date CANINE BIVALENT INFLUENZA, RABIES, DHPP, BIVALENT INFLUENZA, and BORDETELLA VACCINE for the prevention of KENNEL COUGH. Must have Bordetella within 6 months of boarding. Feline patient must have a current RABIES, FVRCP and testing and vaccination against FELINE LEUKEMIA is strongly suggested.
All animals not on heartworm prevention MUST have proof of a purge deworming or negative fecal test within 6 months of boarding. If fecal exam is positive for any parasites all animals will be treated at owner’s expense. This is to ensure the health of your pet and other patients in the hospital.
Any pet not on flea prevention, will be given a Capstar as a flea treatment at the time of boarding. This charge will be added to your invoice. Any animal boarded 5 nights or longer will receive a mandatory bath at owner's expense.
Owner's are responsible for bringing their own food. Bedding and bowls will be provided.
In case of injury or illness, you will authorize VIP Animal Hospital to treat, prescribe, or operate on your pet should the situation dictate. VIP Animal Hospitals will use all reasonable precautions against illness or injury, but is not liable for any illness or injury that occurs while being boarded.
